Health Maintenance Option Plans
Conditions of Enrollment
Thank you for considering one of the preventative health care packages offered by Coastal Equine. The annual enrollment period begins January 1 and ends on April 15. The plans begin once payment has been made and expires on December 31 of the calendar year.
1. Payment for the HMO's is required at or prior to the time of service. We do not offer billing for the packages. You may send payment in advance, call in a credit card or have payment at your spring vaccination appointment. The deadline for enrollment is April 15. If your appointment is scheduled later than April 15, you must provide full payment by the April 15 date. We will not retroactively apply an HMO to spring services previously rendered.
2. Client accounts must be paid in full prior to enrollment in any preventative health care option. If you are sending a check for payment of your HMO, please specify your horse's name and which option you are choosing in the Memo portion of your check.
3. Your account must remain in current financial status in order to receive HMO benefits.
4. Farm calls are NOT included in the HMO's. There is a minimum farm call to all clients regardless of boarding circumstances or location. Split farm calls are divided between the number of clients per farm per call.
5. It is the horse owner's responsibility to schedule all services provided in an HMO. This includes signing up for a barn vaccination visit. If you do not utilize all of the services included in your selected option, there are no refunds or substitutions (use it or lose it).
6. If a horse is sold or dies or you move out of our area while enrolled in an HMO, a pro-rated refund will be given based on the remaining unused services. You can also transfer the package to another horse.
